New Patients

Your first visit to Dr. Daniel Methuselah's office involves a few steps so that we can get to know you. During the initial visit, we will need to obtain important background information, so we request that you bring the following items to your appointment:

  • Photo ID
  • Health insurance or Medicare card
  • List of all current medications
  • List of all known allergies
  • Results of diagnostic studies related to your current foot and ankle problem (i.e., blood tests, bone scans, CT scans, MRIs)
